Health equity is a key component of the live well san diego vision, as well as a longstanding practice in public health services (phs). the newly developed equity framework includes the five areas of influence of the live well san diego framework but is expanded by including additional measures of social determinants of health (sdoh), such as income, housing status, and access to healthcare.
Health Equity Live Well San Diego Live well communities was launched in 2016 focusing on historically underserved areas of the county beginning in the communities of southeastern san diego, as well as nearby locations in lemon grove, spring valley, and national city. the goal of live well communities is to address long standing inequities, disparities, and disproportionality. Partners working towards health equity. effectively removing the barriers that prevent health equity require the work of a community. with over 500 partners and counting, live well san diego has partners working towards all aspects of health equity. see below for the various ways that local organizations are helping their communities.
